Oral Semaglutide vs Foundayo: Comparing the GLP-1 Pills for Women
There are now two GLP-1 pills approved for weight management in the US. One is a peptide with strict morning rules; the other is a small molecule you can take any time. Here is how they compare on the details that matter to women.
The two GLP-1 pills approved in the US for weight management are the Wegovy pill (oral semaglutide 25 mg, from Novo Nordisk) and Foundayo (orforglipron, from Eli Lilly). Both are once-daily tablets. The biggest practical difference is how you take them: the Wegovy pill needs an empty stomach, a small sip of water and a 30-minute wait before eating, while Foundayo can be taken any time, with or without food. The Wegovy pill's main trial reported larger average weight loss, but the trials differed in length and design and never compared the two directly. For women, birth control, pregnancy planning and side-effect tolerance often matter as much as the headline number.

How you take them: the daily-life difference
Wegovy pill: a morning ritual
Semaglutide is a peptide, and peptides are easily broken down in the gut. The tablet uses an absorption enhancer, and absorption is sensitive to food and liquid. The label's instructions: take it with water only, up to 4 ounces, and wait at least 30 minutes before eating food, drinking other beverages or taking other oral medications. That's also why the tablet dose (25 mg) is far higher than the weekly injection (2.4 mg): only a small fraction is absorbed.
For many women, that means taking it on waking and holding off on coffee, breakfast and morning pills for half an hour. If you take thyroid medication, which often has its own empty-stomach rules, or other morning medications, the timing needs planning with your prescriber or pharmacist.
Foundayo: fewer rules
Orforglipron isn't a peptide, so it doesn't need the same protection from digestion. Lilly's announcement says it can be taken once daily at any time, with or without food, with no restrictions on food or water. Tablets should be swallowed whole. If a dose is missed, take it as soon as possible, but never two in the same day; if you miss 7 or more days in a row, contact your prescriber about restarting. Our guide to taking orforglipron walks through the label in more detail.
For women juggling school runs, shift work or irregular mornings, that flexibility may decide it on its own.
What the trials showed, and why you can't compare them directly
The OASIS 4 trial of the 25 mg Wegovy pill reported 13.6% average weight loss at 64 weeks versus 2.2% with placebo, counting everyone randomized whether or not they kept taking it (the treatment-policy estimand). Among people who stayed on treatment, it was 16.6% versus 2.7%. Healio reported that participants on the pill were 12.2 times more likely than placebo to lose at least 20% of body weight.
The ATTAIN-1 trial of Foundayo reported 11.1% average weight loss at the highest dose versus 2.1% with placebo at 72 weeks, counting everyone regardless of whether they finished (the treatment-regimen estimand). Among people who stayed on treatment (the efficacy estimand), it was 12.4% versus 0.9%.
It's tempting to line those up and pick the bigger number, but:
- The trials ran for different lengths (64 vs 72 weeks) in different populations.
- Placebo groups lost different amounts, which hints at different trial conditions.
- Compare the counting-everyone figures with each other; on-treatment figures flatter any drug.
- No trial has compared the two pills head to head.
A fair reading: the Wegovy pill's results look closer to injectable semaglutide's, and Foundayo's look somewhat lower, but your individual response can differ from either average. Neither is a "weaker" choice if you'll actually take it consistently.
Side effects: similar family, a few differences
Both are GLP-1 receptor agonists, so the side-effect profile is broadly similar: nausea, constipation, diarrhea, vomiting and indigestion are the most common, especially when starting and after dose increases. Healio reported that the Wegovy pill's safety profile was similar to injectable semaglutide. Lilly lists nausea, constipation, diarrhea, vomiting, indigestion, stomach pain, headache, bloating, tiredness, belching, heartburn, gas and hair loss among Foundayo's common side effects.
Two points for women specifically:
- Hair shedding is reported more often in women on GLP-1 drugs. See our guide to GLP-1 hair shedding.
- Gallbladder events are listed for both classes. The Wegovy label reports cholelithiasis in 2.5% of people on the tablets versus 1% on placebo. Women already have a higher baseline risk of gallstones.
Both carry the GLP-1 class warning about thyroid C-cell tumors in rodents; neither should be used by people with a personal or family history of medullary thyroid carcinoma or MEN 2.
Birth control, pregnancy and breastfeeding
Lilly says Foundayo may make birth control pills less effective, and another method may be recommended for 30 days after starting and for 30 days after each dose increase. For the Wegovy pill, ask your prescriber and pharmacist how it interacts with your specific contraceptive; the oral semaglutide tablet's strict absorption rules also affect what else you take in that first half hour.
For pregnancy, the Wegovy label advises stopping at least 2 months before a planned pregnancy. Lilly states Foundayo may harm an unborn baby and that breastfeeding is not recommended during treatment. Weight loss can restore ovulation in women whose cycles were irregular, so contraception deserves attention even if you thought fertility wasn't an issue. More in GLP-1s and your period.
Peptide vs non-peptide: does it matter?
On this site we usually write about peptides, and semaglutide is one. Orforglipron is not; it's a small molecule that activates the same receptor. In practice, that's why the rules differ: small molecules survive digestion more easily, so Foundayo doesn't need the empty-stomach routine. For a broader look at why most peptides are injected, see oral vs injectable peptides.
Which pill might suit you? Questions to discuss
- Can I reliably take a pill on an empty stomach and wait 30 minutes every morning?
- What other morning medications do I take (thyroid, iron, calcium, birth control), and how would each pill fit around them?
- Which option does my insurance cover, and what are the self-pay options this month?
- How does each affect my contraception, and for how long after dose changes?
- Am I planning a pregnancy in the next year or two?
- Would an injection be a better fit for me than either pill? (Our semaglutide profile covers the injectable.)
Availability and cost
Novo Nordisk said it expected a US launch of the Wegovy pill in early January 2026. Lilly said Foundayo prescriptions could be filled through its LillyDirect pharmacy from April 6, 2026, with retail pharmacies and telehealth providers following. Both makers run savings and self-pay programs whose prices change; check the current figures with your pharmacy or the manufacturer rather than relying on numbers you saw months ago. Prescription Option (Telehealth)
Telehealth clinicians can prescribe FDA-approved GLP-1 pills when they're appropriate. Ask specifically about brand-name oral options if that's what you want.
Sesame Care offers telehealth weight-care visits with licensed clinicians. The clinician reviews your history and decides whether a medication is appropriate.
